Customized Care

Routine Home Care

Home care is a service provided wherever the patient lives in Maricopa County. We come to the home, not only to provide medical care for the patient, but to offer emotional and spiritual support and comfort to the family as well. This allows the patient to be in familiar surroundings with the ones they love.

Inpatient Care

Inpatient care is provided at an inpatient hospice facility when symptoms cannot be managed at home. A highly skilled team is present for immediate patient care 24 hours a day.

Respite Care

Respite care is offered for periods of rest for caregivers of the patient. It is a way to extend a reprieve to the family during a challenging time.

Continuous Care

Should a medical crisis arise, our staff is here to provide in-home, around-the-clock care. Our skilled nurses bring a sense of peace and comfort while managing the patient’s physical symptoms. We encourage you to call us in an emergency, so we are able to manage your symptoms in the comfort of your own home.

Programs

Bereavement Program

Wings of Hope bereavement counseling is comforting to both the patient’s family and the caregivers. We are able to connect you with a community of others on the same journey, we will be with you for those painful dates and anniversaries, and we will continue to provide the emotional support needed in the most peaceful manner. Wings of Hope will remain part of your family well beyond the care of the patient. Our bereavement team is available to you any time day or night to lift you up. Grieving is a natural human emotion and it is our humble honor to serve you during this painful time.

Grief Support Groups

We all have different ways in which we grieve. Because of this, our support groups offer you several ways to process through your loss. We talk with you to learn more about what kind of loss you are experiencing. Whether you have experienced a death, or you have an ambiguous loss (loss that happens little by little) due to a loved one with dementia or Parkinson’s, you still need a way to process what you are going through. We can offer several ways to help you process these things such as cognitive behavioral therapy, art therapy, poetry, journaling and education on grief and loss. So however you learn, we have an individualized plan to help you get back into your life. Join us so you can find your way back.

Gift of Presence Program

The Gift of Presence Program helps to ensure that no patient is alone by being present at the end of life’s journey. These specially trained Volunteers will be at the patient’s bedside with the patient and/or family at the time of death to provide comfort and support.

Palliative Care Program

Palliative care is a wonderful resource for those who do not qualify for Hospice but need the Wings of Hope touch. We are able to provide loving care any time during the course of an illness in conjunction with treatments and skilled nursing care.

Specialty Programs

Wings of Hope offers these unique programs for patients with severe conditions, including:

  • Congestive heart failure
  • Lung conditions
  • Parkinson’s, ALS and movement disorders
  • Dialysis
  • Oncology

A Team with Hearts to Serve

It is our honor to serve our patients and their families. Our lower staff-to-patient ratio allows us to visit more frequently and enhance personal care.

Skilled Nursing

Wings of Hope provides skilled nurses to coordinate with patient’s care, medication management, observation, and any nursing needs. They are a personalized link to the family in education and care of their loved one. They will help facilitate comfort and serenity for the family.

Certified Nursing Assistants

Our CNAs assist the patient with personal care such as bathing, grooming and dressing, as well as light housekeeping and moral support. They ensure the patient is treated with dignity and respect.

Medical Social Workers

The job of our Social Workers allows our patient and their families to remain focused on the most important thing – time with their loved ones. They help navigate through difficult conversations and provide advice and emotional support when families are dealing with final arrangements, living wills, advance directives, and advocating the patient’s wishes. They also help ensure each patient is utilizing the many benefits of Hospice care, providing resources that guide patients and loved ones through emotional times.

Bereavement Counseling

Spiritual Counselor

Wings of Hope Hospice spiritual care can be provided upon request and is surrounding the select faith community of the patient and their family. We will pray with you by your side, or can help coordinate time for you at your church of choice.

Volunteer Staff

Our dedicated volunteer team provides many resources to envelop our patients and families in loving care. They serve with compassion, companionship and emotional support to both the patients and their family.
