What is Palliative Care?

Palliative care is designed to offer compassionate, whole-person support for individuals facing serious illness—at any stage and alongside curative treatments.

Unlike hospice care, which is reserved for those nearing the end of life, palliative care can begin at diagnosis and continue throughout treatment, helping patients manage symptoms, reduce stress, and improve their overall quality of life.

Your Palliative Care Team

In palliative care, a clinical team collaborates to support the patient’s physical, emotional, and spiritual well-being. This team works closely with the patient and their family to ensure care is aligned with the patient’s goals, values, and needs. Your palliative care team may include:

  • Palliative Care Physician or Nurse Practitioner – Expert in symptom management, helping guide medical decisions based on patient goals and comfort. Provides hands-on care, medication management, and patient education.
  • Medical Social Worker – Offers psycho-social and emotional support. Helps with advanced care planning and connects patients to care resources.

I am writing to tell you what extraordinary, caring, compassionate people we have had caring for my mom. From the initial meeting with Angela to Carol our nurse and the CNA’s Rosie, Cindy, and Michelle. Your hospice/palliative care is unmistakable. Carol advised us every step of the way to make her comfortable. The CNA’s Rosie, Cindy, and Michelle were gentle & caring, as mom told me many times after they left. When she passed, I received a call from Chaplin Elsa a few days later. That call was something that I didn’t know I needed. Her kind words and prayers helped me understand mom’s passing. I truly don’t know how you do this hospice care but thank God for people like you.
